Airlines are required to provide the attestation form to their passengers prior to embarkation.
PASSENGER DISCLOSURE AND ATTESTATION TO THE UNITED STATES OF AMERICA
All airlines operators covered by the passenger disclosure and attestation to United States Order must provide the disclosure to their passengers and collect the attestation prior to embarkation. On Behalf of the US Centers for Disease Control and Prevention (CDC), all airlines are required to confirm either a negative COVID-19 test result or recovery from COVID-19 and clearance to travel and collect passengers attestation, as required by United States Federal Law.
Each individual 2 years of age or older must provide a separate attestation.
